How to apply through the CK Food Bank for food stamps:

  1. Call Kathy at 692-9818 for an appointment.
  2. Bring to the appointment:
  • The name, date of birth and Social Security number of everyone in your household.
  • Contact information (name, address, phone) for your landlord or mortgage company.
  • Picture ID.
  • If employed, paycheck stubs from the last 2 months.
